Practice PCN Staff

Our practice team are supported in delivering services by a wider healthcare team. You may be offered an appointment with one of these roles who specialise in their area of work.

These roles include:

FCPs (First Contact Physiotherapist) – First contact physiotherapists are experts in joint and muscle pain. They can advise exercises, diagnose, request scans, and refer on to the hospital for investigations or support.

Paramedic – Our paramedic is able to deal with infections and other illnesses.

Clinical Pharmacist – Clinical pharmacists are experts in medication – they can review your medication to make sure you are on the right doses and advise of alternative medications.

Pharmacy Technician – our pharmacy technicians work alongside our clinical pharmacists to ensure that medicines are prescribed in line with the most up-to-date NHS guidance.

Mental Health Worker – Mental health workers are experts in Mental Health – they can help you to get the right support for your needs and can refer on to specialist teams if needed.

Social Prescriber – The Social Prescribing Team are available for support with non-clinical needs. They can support with debt, employment, housing, mobility issues, loneliness, money worries, and much more!

Care Home Team Advanced Nurse Practitioner (For care home patients) – The care home team’s Advanced Nurse practitioner supports local care home with patients’ health needs and regularly meets with care homes to ensure that the best care possible is provided.

Acute Home Visiting Service Paramedics/Advanced Nurse Practitioners (For housebound patients) – The Acute home visiting team are a team of highly skills nurse practitioners able to prescribe for and diagnose health problems for patients who are physically unable to get to the surgery.
