Accountable / Named GP for All Patients

From 1 April 2015, and before the 31st March 2016, practices are required under the contract to allocate a named, accountable GP, who is responsible for overall care at the practice, to all patients (including children). Middleton Lodge has always operated a personal, named GP list and therefore if you are registered with the practice you will already have a named accountable GP. For those patients who express a preference as to which GP that is the practice will make every reasonable effort to accommodate the request

New patients joining the practice will be allocated a named GP at the point of registration.

If you would like to confirm who your named accountable GP is please ask the receptionist when you next visit the surgery.

Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP may not always be available and, if your needs are urgent, you may need to discuss them with an alternative doctor.

The practice can confirm that all patients registered with the Middleton Lodge Practice have been allocated a named accountable GP
